If you have no idea what's going and you are being attacked, you might not get any signal before. But most of the time, you will see some sort of signal or indication that you are going to be attacked.

Every situation is different and you cannot fit one thing in every situation. What works in one situation, that might not work in another situation. When you fight for self defense, your move should be based on the situation.
Situational awareness helps you prepare to face the situation and make you ready beforehand. When you are in a place, you see something is wrong, someone has an argument, that might not something serious. But still, you might get attacked because of that.
The attacker is not stronger and bigger than you are. Please do not take it for granted. If the attacker has a weapon or comes out with a weapon, he can have a 10x advantage than you are. So never underestimate your opponent, otherwise, you have to pay the price.
Let's see a real-life situation that actually happened. This is not my video, and video credit goes to the Fight SCIENCE. Let's watch this vidoe.
In this video, you see one guy coming out with a baseball bat and attacks. If you watch carefully, you can see what happened before. The attacker, the little guy, have an argument. He could not stand a chance over there. The big guy totally dominates that.
Then the attacker went back and came out with a baseball bat. Here the early argument and the way the attacker is talking is a sign. So when it was seen the attacker was running towards him grabbing a baseball bat, he would surely hit the guy with that.
The guy was talking on his phone. When you saw that, he moved forward and made his move to defend himself and did it successfully. Situational awareness and a fast reaction to the situation can change the whole thing.
